Patient Portal
The easiest way to pay your ProActive bill is through their online patient portal You can access the portal at payproactiv.com/user/login.
If it’s your first time, click “Create an Account” and provide your name, email, date of birth and account number from your bill. This sets up your login credentials.
Once registered, you can log in to view your account balance, previous statements and make one-time payments on your bill. It’s simple, fast and can be done 24/7!
Payment Options
ProActive offers several payment options when you pay your bill through their online portal or website:

Credit/Debit Card – Pay quickly by Visa, Mastercard, Discover or American Express.

HSA/FSA Card – Use your pre-tax health accounts funds to pay.

Bank Account – Securely pay from your checking or savings account
Select your preferred payment method each time you pay online. Credit cards and bank accounts can also be saved for future transactions.
Partial or Full Payment
Depending on your current balance owed, you can choose to make a full or partial online payment to ProActive.
To pay in full, simply enter your total account balance due as the payment amount when checking out. This zeros out your account.
Or enter a smaller amount than your total balance to make a partial payment instead. You can pay the rest later in additional installments.
Payment Receipt
Every time you make an online payment, you’ll receive a receipt via email with payment confirmation details.
The receipt will show the payment date, account number paid, amount paid, confirmation number and other details.
Save these receipts or print them for your records in case you ever need to reference them.
Automatic Payments
For hassle-free bill pay, consider setting up automatic payments through the ProActive patient portal. This automatically charges your card on file each month for the balance due.
To enroll, indicate you want to auto-pay when checking out. Your card will then be charged monthly going forward on the due date until you cancel.
Account Statements
In addition to making payments, you can also view PDF statements for your ProActive Physical Therapy account by logging into the patient portal.
Statements outline your visits, charges, insurance coverage, payments made, and current balance due. Review these periodically to verify their accuracy.
Paperless Billing

Similarly, if you have a health savings account (HSA), you can use these funds to pay your ProActive bills tax-free.
Just use your HSA debit card or bank account on file when paying online. HSAs help you maximize savings.
Itemized Receipts
Request an itemized receipt from ProActive anytime you need documentation for insurance reimbursement or your records.

How to set up Bill Pay
- Load money to your Payactiv Visa® Card* We recommend setting up Direct Deposit before Bill Pay so that your money is in one place, all viewable in the Payactiv app. If you don’t set up Direct Deposit, you’ll have to manually transfer funds from another account to cover your bills.
- Add a biller and pick a payment date From the Bill Pay screen, tap “add a biller”. Next, select a date you want your payment to go through. We recommend 3 days before the due date, just in case!
